Online Registration (secure):

  • If you are 16 years of age or older and a Massachusetts resident, work in Massachusetts or own property in Wellesley, you may apply online for a library card.
  • Fill in and click on the Submit button.
  • Your card will be ready for pickup at the Main Library within 3 business days.
  • You will be notified by e-mail when your card is ready.
  • If you provide and e-mail address, information about the arrival of requested items and your library account will be e-mailed to you.
  • We do not mail library cards.
  • To activate your card
    You must bring in an acceptable form of identification (such as a driver's license) to the Circulation Desk at the Main Library .  Library cards not picked up within 30 days are expired
  • In-person registration or under 16 years of age:
    Fill out this form, print it and bring to the Main Library with photo identification, such as a valid Massachusetts driver's license or passport and verification of address such as cancelled mail or utility bill.  If photo identifcation inculdes your current address, it is the only identifcation needed
  • For children under the age of 16, this form must be signed by a parent or guardian.
  • Please Note: If you have had a Minuteman Library Card in the past 5 years, please do not submit a form online. We already have you on record. Come to the Main Library with a valid form of Identification and we will issue you a new library card.
